Fixed. However, this server is still being monitored.
We suspect a power supply is acting up and done a bit to check it tonight, we will monitor it close and do a swap on weekend if needed.
MySQL6 is again having an issue and the problem has been found, we will schedule an outage within the next 36 hours to swap the power supply, but working to bring it online right now.
For now, server is up. It was a kernel panic issue which we did fix.
