Email questions

One of my clients has noticed some seemingly odd occurrences with their mail (mail3 server) and I am wondering if anyone else has seen these behaviors?

1) One of their mailboxes had almost 2,000 spam messages in it after a few days, most all of it to his info@ mailbox. Some were marked as spam by SA and some were not.

2) Today, he received an email from a postmaster at another domain which indicated that his info@ account had sent an undeliverable email, although it is not used to send email. It was some Canadian drug spam. It showed the info@ as an authenticated SquirrelMail user. How is this possible? What can be done about it?

Any input on the above would be appreciated.
