I see that your 14.95 package includes 1 free mysql database but then it says $5/month. Does this means I have to pay an additional 5 bucks a month (19.95) to use the database? Also, are your prices in US dollars? Thanks.

One database is included in the package price. $5/month is for additional database. All prices are in US dollars.
