slow asp form

[CASE]

I have an ASP form that worked perfectly at my old host. Now there is a huge delay after hitting ?submit?. When it finally goes through, the results are emailed and submitted into an access database. http://www.woodstocksiv.com/survey/login.asp?surveyid=delivery

Do you know why it would be so slow?
